Gun issue and how did you solve it?

Can you tell us about a time when you encountered a reliability problem and how you managed to resolve it?
 
I had a compact 9mm that kept failing to feed and everyone online blamed the gun itself.. but it turned out my grip was bumping the slide stop. I adjusted my thumbs and...problem gone. So, not every issue is the gun's fault.
 
The worst problem I ever had turned out to be something simple like the choice of ammo. Once I switched that up, the problem completely went away. That taught me to check the basics before blaming the gun.
 
I put together a PSA AR that the upper was OK, but when I bult the lower it wasn't accurate at all. I had trouble putting it together because some parts were out of spec. I finally realized that the Bolt or BCG was out of spec and binding causing in consistent chambering. I got a BCA BCG and it shot great.
